After the form is completed, sign and fax it with a copy of the patient’s insurance information to 866-329-2224.
Castle will then initiate a benefits investigation and coordinate specimen collection with the physician’s staff and hospital.
Patient results will be sent to the physician(s) indicated on the requisition form.
